Root canal treatment

The primary teeth form the most important guideline for the newly developing permanent teeth that will replace them eventually. Root canal treatment can preserve tooth, jaw and tongue function of the child. It can also prevent speech problems, or even the loss of a tooth that has no successor. The diagnosis is complicated and, therefore, appropriate treatment is required than dealing with permanent teeth.

Nowadays various state-of-the art treatment techniques are available for children such as:

Indirect pulp treatment

For teeth with deep decay approaching where removing all the decayed parts of the tooth would expose it this method is used. The soft decay present is removed as far as possible, leaving only harder remnants without penetrating into the pulp. An antibacterial agent is applied and restoring the tooth to seal it prevents further infection. The outer layers of infected portion are removed and a layer of lining cements is placed over the exposed part. A temporary filling is then placed in the tooth to ensure comfort and healing. After 10-12 weeks, a permanent filling is placed.

Direct Pulp Capping

This is recommended for small incidental exposures of the pulp when there is no decay. Here, the dentist caps the exposure directly using similar materials as mentioned above, to create a bridge to seal the exposure.

Pulpotomy

Pulpotomy is nothing but partial pulp removal. It is used to treat pulp exposures, it occurs as a result of decay in primary teeth, when the infection is confined to the coronal (inside the crown) area of the pulp. The procedure involves removal of the coronal portion of the pulp, ensuring that the vitality of the remaining root areas of the pulp is preserved. Effective control of infection is also crucial so as to prevent spreading.

Pulpectomy

This is complete removal of the pulp tissue as it is totally damaged by infection and recovery is not possible. The procedure is similar to traditional root canal treatment like disinfecting, cleaning, shaping and filling of the canals to seal them. Only the sealants used are absorbable so that roots are naturally absorbed. After completion of root canal treatment for primary teeth, the restoration of choice for a back tooth is a stainless steel crown and, for a front tooth, a composite tooth-colored resin.
